Welcome to The Path Center for Integrative and Functional Medicine Patient Portal. We are so excited you decided to join our wellness family and get on The Path to better health. Once you have scheduled your Initial Consultation, you can log in to the portal to complete the onboarding process and access your records.

To complete the onboarding process, you will need to provide your emergency contact, preferred pharmacy, insurance, allergies, and vaccinations. We also ask that you please provide your photo. Once this basic information is complete you can move on to the “Onboarding

Documents” found under the “Forms and Questionnaires” tab of your portal. We require you sign and date all administrative forms and complete the required intake questionnaires one week prior to your scheduled appointment.

PAYMENT: payments for appointments, test kits, and/or supplements are due at the time of service. Telemedicine calls will be charged to the credit card we have on file following your appointment.

CANCELLATIONS: Cancellations less than 48 hours of your appointment time will be charged the full appointment fee. Please understand that when you cancel with little notice we are unable to offer that time slot to someone else in need and cannot make up for the lost revenue.
